The work mapped case manager workflows, analyzed how operational steps relate to patient experience and safety, and prototyped dashboard concepts that made coordination activities visible. What I've accomplished

I produced an interactive stakeholder map and accompanying table that documented roles, care pathways, coordination and referral paths, and points where operational support links to patient care.

I mapped stakeholders in mental health case management, listed their roles and care pathways, and produced an interactive visual and table that showed coordination, referral paths, and where operational support (IT, leadership) connects to patient care.

I reviewed AI-identified issues, mapped observable failures across discharge, medication access, communication, and follow-up, and recorded affected parties, hidden risks, and internal process gaps. The work produced a shared spreadsheet of countable workflow gaps and risks.

I compared official mental health and medicines guidelines with a clinic case. I identified a discharge gap—three days of medication vs a seven-day wait—and traced the internal process failures, then recommended a pre-discharge medication check and clearer follow-up ownership.
