Workflow analysis & dashboard creation
I performed an analysis of case management workflows to identify an issue to address. I focused on patient disengagement from care continuity plans during the initial post-discharge phase and created an interactive dashboard that attempts to mitigate the issue.

Overview

The project analyzed case manager responsibilities, mapped stakeholders and workflows, and prototyped a dashboard to quantify case-management work. Deliverables included a scope-of-work spreadsheet, failure-mode documentation, Mermaid flowcharts of outreach and escalation, and a live dashboard prototype. The work was awarded Top Performer by Institute of Mental Health.

I produced a scope-of-work spreadsheet, documented priority failure modes for high-risk patients, created Mermaid flowcharts of outreach and escalation, and delivered a live work-quantification dashboard prototype with eight defined metrics.

Overview

What I've accomplished

I started by researching case management roles and best-practice benchmarks, then performed root cause analysis using a fishbone diagram (Ishikawa) and 5 Whys questioning. Later, I used an example case to help create a decision tree for case managers. The project culminated in the creation of a dashboard informed by all of the previous work. I used Claude, ChatGPT, Mermaids, Google Sheets, & Miro

Outcome

The dashboard provides proactive/reactive benefits through monitoring & tracking, enhanced visibility, consistent messaging, improved information sharing, clear ownership of tasks, and specialized functions adapted to each stakeholder. Overall, this tool creates greater efficiency in processes while clearly showing each staff member what needs to be prioritized at any particular moment.
