During my externship at the Institute of Mental Health, I focused on the design and analysis of care coordination in acute mental health settings. I examined the crucial role of case managers, uncovering how workflows affect patient experiences and identifying risks within these processes. My work included process mapping and prototype dashboard creation, aimed at enhancing operational clarity
I produced a stakeholder map and a mapped case manager workflow, and applied root cause analysis methods to identify underlying issues, documenting multiple insight moments and reflection on my analytical development.
The module asked for a stakeholder map and role summary. I mapped the case manager role, listed key responsibilities, daily workflows, pain points, and cited supporting quotes and sources. The submission produced a detailed scope-of-work spreadsheet.
I collected observable issues in clinic workflows, listed who was affected, the hidden clinical risks, and the internal process gaps. The submission produced a spreadsheet of six concrete failure modes, each paired with the responsible actors and specific workflow breakdowns.
