What I've accomplished

I produced a detailed scope-of-work spreadsheet that mapped case manager responsibilities, daily workflows, pain points, direct quotes, handoff gaps, follow-up failures, and access barriers such as transport and waitlists.

Project breakdown

The project analyzed the case manager role in mental healthcare, mapped daily tasks and workflows, documented pain points (missing discharge info, transport barriers, scattered records), and produced a scope-of-work spreadsheet with cited sources and role-specific entries.

I mapped observable failures across clinic workflows, noting who was affected, hidden risks, and internal process gaps. I identified missed follow-ups, medication access gaps, unnoticed missed appointments, and eroded trust, and recorded each issue in a shared spreadsheet.

The project examined missed follow-up appointments, mapped a decision tree for case manager contact, booking checks, and escalation, and produced a concise Mermaid.js flowchart that clarified each path and decision point.
