What I've accomplished

I produced a live stakeholder map and a written summary that described case manager workflows, their coordination role, and communication gaps that delay care.

Project breakdown

While mapping stakeholders, I documented the case manager's core functions, daily workflows, triggers, and pain points, then produced a role-summary spreadsheet linking continuity, risk surveillance, information integration, and crisis coordination to specific tasks and evidence.

In the externship, I catalogued discharge, medication, and communication breakdowns that create patient risk, listed who is affected, noted hidden risks, and recorded specific internal process gaps in a shared spreadsheet; the sheet produced a structured inventory for root cause analysis.
