This externship explores how care coordination work in acute mental health settings can be designed, analyzed, and communicated as a customer-centric operational system. Externs study the case manager’s role at IMH to understand how workflows shape patient experience, surface hidden risks, and make invisible coordination work visible. Through structured analysis, process mapping, and dashboard prototyping, externs learn how operational clarity supports continuity of care, safety, and institutional decision-making.
The project examined care coordination and the case manager role at the Institute of Mental Health, producing process maps, root-cause analyses, a decision-tree target process, and a pilot work-quantification dashboard prototype. The work was awarded Top Performer by Institute of Mental Health.
I produced a stakeholder map of the case manager's five role 'hats', traced 20 patient touchpoints, classified tasks by automability, completed Fishbone and 5 Whys analyses, built a decision-tree target process with an UNCONFIRMED state and escalation logic, and created a pilot dashboard prototype.
