The project analyzed case manager workflows and produced artifacts to make coordination work visible, including a stakeholder map, scope-of-work sheet, workflow failure analysis, decision trees, visual flowcharts, and a dashboard prototype. The work was awarded Top Performer by Institute of Mental Health.

What I've accomplished

I produced a stakeholder map and scope-of-work sheet, an issues table linking failures to roles and process gaps, a decision tree and visual flowchart for high-risk appointments, and a care-coordination dashboard prototype surfacing high-risk patients and assigned actions.
