    Healthcare Operations & Consulting Externship

This externship explores how care coordination work in acute mental health settings can be designed, analyzed, and communicated as a customer-centric operational system. Externs study the case manager’s role at IMH to understand how workflows shape patient experience, surface hidden risks, and make invisible coordination work visible. Through structured analysis, process mapping, and dashboard prototyping, externs learn how operational clarity supports continuity of care, safety, and institutional decision-making.

Overview

The externship examined care coordination work in acute mental health settings by mapping the case manager role and analyzing operational workflows. The work combined process mapping and dashboard prototyping to make coordination activities visible and to identify workflow risks. Deliverables included workflow maps and a prototype dashboard reflecting coordination tasks.

What I've accomplished

I produced two visual maps and a role table that documented coordination tasks, communication gaps, and the case manager's responsibilities in a mental-health case.

Project breakdown

The module presented a mental-health case; I mapped stakeholders and summarized the case manager’s responsibilities, documenting coordination tasks, communication gaps, and opportunities for clearer role boundaries. I produced two visual maps and a role table.

I identified that patients repeating their stories stemmed from poor information sharing. I mapped causes across people, process, technology, records, resources, and environment, and proposed a standard handover, updated records, and shared access as solutions.

I analysed guidelines and IMH workflows, wrote a decision tree for preventing patients from repeating their stories, and produced a visual flowchart that mapped handover steps, acknowledgements, follow-up calls, and escalation paths to clarify ownership and timing.
