The project analyzed care coordination and case management workflows at an acute mental health provider, documenting how tasks, handoffs, and communication channels operated across the patient pathway. The work mapped processes, identified operational touchpoints and risks, and produced dashboard prototypes to visualize coordination and measure key workflow steps.
