The externship examined case manager workflows in an acute mental health setting, using process mapping and comparative analysis to reveal how operations shape patient experience and safety. Work included assessing communication and efficiency factors across service models, and prototyping dashboard concepts and process artifacts based on those analyses.

What I've accomplished

I produced a stakeholder map and a shared spreadsheet inventory that listed daily tasks, required information, and highlighted overlapping responsibilities in a mental health case-management workflow.

Faced with blurred role boundaries in a mental health case, I mapped stakeholders, listed their daily tasks and required information, and noted overlapping responsibilities. The deliverable produced a shared spreadsheet visualizing roles and workflow gaps.

I mapped patient and internal workflow gaps, identified 'next steps unclear' as the core problem, built a Fishbone and 5 Whys analysis, and produced recommended fixes (communication protocols, role RACI, centralized documentation).

A discharge-to-follow-up decision tree was created for case managers. I mapped system alerts, structured handoffs, timed reminders, contact attempts, attendance checks, and risk-based escalation steps into a stepwise workflow, producing a complete, timestamped process.
